First time consultations or appointments
If you are scheduled for a first time consultation or appointment, be sure to fax the following to your office location and/or bring to your first visit:
- Pertinent Medical Records
- Radiology reports and films
- Pathology reports and slides
- Medical insurance card(s)
- Insurance referrals from primary care physicians. This information MUST be available at the time of the first appointment.
